Tuberculosis Meningitis Associated with Poncet’s Disease: A Rare Case in a Young Senegalese Patient

Described by Antonin Poncet in 1897, Poncet’s disease is a non-destructive (non-erosive) inflammatory arthritis secondary to mycobacterial infection, without an identified infectious agent the affected joints. Neuromeningeal localization of tuberculosis (TB) rare but remains most severe with hight mortality and permanent neurological damage. Association rheumatism TB rare, even less neuromeningeal localization. We report here case optic nerve compression syndrome complicating tuberculous meningitis associated characterized good outcome. This was 18-year-old female no reported pathological history, who admitted for obnubilation. The symptoms started two weeks before her hospitalization were marked fever headache. On admission, examination revealed obnubilation, bilateral pyramidal deficit syndrome, right ptosis areactive mydriasis predominating on right, meningeal intense polyarthralgia. brain CT showed contrast enhancement moderate dilatation ventricles. GeneXpert MTB/rif CSF positive. Rheumatoid factors (latex Waaler-Rose test) anti-CCP antibodies negative. Anti-nuclear, anti-native DNA anti-NAS Ophthalmological extrinsic intrinsic oculomotor paralysis eye. Orbito-encephalic MRI nerve, hydrocephalus. Under antituberculosis treatment, we noticed disappearance arthralgia three weeks, regression intracranial hypertension improvement vision.
